Reminders from the Health Office:
- New students must have a Comprehensive Physical on file in the health office within 30 calendar days.
- Please return the Confidential Annual Health Report Update form to the health office. In order for your child to receive Tylenol and/or Ibuprofen in school this form must be completed with a parent/guardian signature.
- If your child needs to take any medications in school, including over the counter medications, an Authorization for Medication Administration form must be completed and signed by the parent/guardian and prescribing physician. Please note, this form is not needed for Tylenol/Ibuprofen.
- The Board of Education and the State of Connecticut requires all 6th and 10th grade Comprehensive Physicals and immunizations to be on file in the health office. If your child is playing sports, a recent physical is required. Sports physicals are valid for 13 months from the date completed. Please note: In order to participate in try outs for WMS/WHS teams, this must be on file in the health office.
- Forward any updated immunizations received to the health office.
- Forward any physician directives needed to assist your child throughout the day. It is the parent/guardians responsibility to notify the nurse and forward Food Allergy Treatment Plans and Asthma / Allergy Plans with physician directives. Please notify the school nurse if your child requires any health modifications, diabetes protocols, severe peanut/bee allergies, psychological support, etc.
- If your child is going to be out ill, please notify the main office and/or nurse.
- Health Screenings will be done by the nurse during the school year.
- Vision: 5th, and 9th grade
- Hearing: 5th and 8th grade
- Scoliosis: 5th ,7th, 8th, 9th grade Note: 6th and 10th grade should receive vision,
hearing, and scoliosis screens at their required Comprehensive Physical.
