  • Immunizations: Please review the State Immunization requirements paying close attention to those for Preschool & Kindergarten.  These are available on the link below.  Please forward any updates on immunizations to the nursing office.  
  • MedicationIf your child needs any medications while at school an “Authorization for Administration of Medication” form needs to be completed & signed by you and your MD.  These forms are available here & at your Pediatricians office.  You will need to supply all medications ordered. Students are not permitted to carry medications to/from school to the nursing office. Students are not allowed to carry medications on  self unless otherwise approved by physcian, parent & school nurse. 
  • Yearly Physical:  Preschool & Kindergarten students are required by the State of CT to have a complete physical examination.  A copy of this examination needs to be brought to and on file in the health office.  It is encouraged to have a copy of the students yearly physical sent in for other grades as well (post Elementary required exams include grades 6 and 10). 
  • Physician Directives:  Please forward any physician directives needed to assist your child during the day.  These include but are not limited to Allergy Treatment Plans & Asthma Treatment Plans.  Please inform the nurse if your child may need any health/activity modifications, diabetes protocols, psychological support, etc.  For example, If a student activity needs to be limited due to injury we request physician directives regarding what type of limitations & when the child is allowed to return to normal activity. 
  • Illness: When students have a fever (>/= 100.0) they are considered contagious & should remain home until they are fever free for 24 hours w/out the use of a fever reducer.  If a student is vomiting they should remain home for 24 hours free of vomiting before they may return to school.  If either of these occur while in school your emergency numbers will be called & student will be dismissed.
  • Students Absence:  If your student will be absent please call the office in the AM & specify type of illness or vacation. 
  •  Health Insurance:  If your child or family is not protected by Health Insurance & you would like information regarding the CT Husky Insurance Plan call 1-877-CTHUSKY.  You may apply by phone, request an information kit or visit  www.huskyhealth.com.

Immunization Changes for 2011-2012
